Fellowship Reinstatement Request
Students with fellowships who return from an approved leave of absence should submit this form preferably six weeks before the start of the semester in which they wish to return.
Once the student submits the Request for Fellowship Reinstatement form, it will route to the department and the Graduate and Professional School for approval. The student will be notified once the request has been approved.
