Graduation Cancellation Form
Complete this form to cancel your application for graduation. Be aware that university guidelines state that students who have completed all degree requirements will not be allowed to cancel their graduation.
After filling out all applicable information, the contact information for your committee chair must be submitted on the next page.
If it is after the last day in the semester for all students to drop courses with no penalty (i.e.,Q-drop), contact information for the Department Head or Intercollegiate Chair must be included as well.
